Standing With Families Through Dementia: How At Your Home Familycare Is Expanding Support Across
San Diego
By Laurie Edwards-Tate, MS
If you’ve ever loved someone living with dementia, you already know — this journey changes everything. It reshapes family dynamics, stretches emotional and physical limits, and often leaves caregivers feeling exhausted, isolated, and unsure where to turn for help.
At Your Home Familycare has walked alongside thousands of San Diego families over the past 40 years. We see the behindthe-scenes reality every day — the spouses running on little sleep, the adult children juggling work and caregiving, the quiet stress that builds when support feels out of reach.
That’s why I’m incredibly proud to share that At Your Home Familycare has been selected as an official Respite Care Partner with Tembo Health as part of Medicare’s new GUIDE Dementia Care Program — Guiding an Improved Dementia Experience.
And more importantly, why this matters for families right here in San Diego County.
Dementia Isn’t Just a Diagnosis — It’s a Family Crisis
Dementia is one of the fastest-growing health challenges in our country. As people live longer, more families find themselves stepping into full-time caregiving roles — often unexpectedly.
What begins as helping with a few tasks can quickly become 24/7 care. Caregivers are managing medications, meals, safety concerns, appointments, emotional changes, and daily routines — all while trying to hold onto their own health, careers, and family responsibilities.
Stress can be overwhelming. Burnout is real. And too many caregivers try to do it all alone.
That’s exactly what the Medicare GUIDE Program was created to address — improving care coordination, strengthening caregiver support, and making respite care accessible for eligible families at no cost.
For many families, this includes up to approximately $2,500 each year in covered respite services — real relief, not just advice.
How This Partnership Helps Families Breathe Again
Through this program, Tembo Health serves as the clinical care partner — providing dementia evaluations, medical oversight, education, and ongoing coordination. At Your Home Familycare steps in where daily life happens: inside the home. Our caregivers provide hands-on respite support, including:
• Personal care and mobility assistance
• Companionship and supervision
• Meal support and daily living help
• A safe, compassionate presence families can trust
While caregivers take time to rest, work, attend appointments, or simply recharge, their loved ones receive professional care in the comfort of home. And that combination makes all the difference.
Why This Means So Much to Us
We see firsthand how dementia reshapes entire families — emotionally, physically, and financially.
Caregivers are doing heroic work behind closed doors. They love fiercely, give endlessly, and too often, they’re exhausted. This partnership allows us to step in with real, tangible support. Not just words of encouragement, but hands-on care that gives families breathing room, peace of mind, and renewed strength to continue the journey. Because dementia isn’t just a medical condition.
It’s a family experience. And no family should have to navigate it alone.
